One thing that's worried me about McCarthy in the last few games has been his non-existence in the lineout. Haven't got stats, but I'd wager he's far behind POM and D. Ryan in terms of lineout takes. For every knee ligament injury you get, the probability of that injury reoccuring increases ten fold with every one. Trust me, this is coming from a man who's career was cut short because of knee injuries that if they never occured, O'Gara would only have about 6 caps for Ireland. ;) Perhaps clinic...

It's very much unlikely that an ACL injury would have occurred because of older injuries. Much more likely that it was an awkward fall or tackle. Odds are his previous history has got nothing to do with that.
